Board Forms

IMPORTANT NOTIFICATIONS

As of July 15, 2025, the documentation that will be submitted to the Department of Public Safety has been updated. The application for initial licensure reflects those changes. Please make sure that the Originating Agency Identifier (ORI) is NV920734Z on your fingerprint forms. If the correct ORI number is not used, the fingerprint submissions will be rejected.

In addition, the total fee for a fingerprint background check will be $39.00, effective January 1, 2025. The cost of the FBI fee portion of the fingerprint background checks was decreased, while the State fee portion remained the same. The Department of Public Safety will not accept payment made out to the incorrect amount.

Please Note: Nevada does not offer any type of temporary or provisional license.

As of June 20, 2024, the Board has reduced the fees for an initial license to $200.00 and the annual renewal fees to $125.00. The Board also discounted the initial licensure fees for active military, veterans, and their spouses. Recent graduates of the UNLV Athletic Training program may also qualify for discounted licensure fees. Please see the NSBAT Fee Schedule (below) for more details.

ALL licenses to practice athletic training expire on June 30th each year, regardless of the date they were issued. The online renewal application will be available online beginning May 1st of each year.

Lastly, the Board is required to keep a public address on file for each licensee. Your work address is used as your public address by default. If you do not provide the Board with a work address, your home address will be used as your public address. You may provide an alternate address by emailing the Board office.

              Fingerprint Background Checks

Results of fingerprint background checks are taking at least 30 - 45 days even when submitting via live scan.  Licenses cannot be issued until the results of the fingerprint background check are received.

Live scan is available for applicants present in Nevada only. The Live Scan Private Fingerprint Sites form below has a list of approved fingerprint sites provided by the Nevada Department of Public Safety (DPS) that have met the technical security requirements to electronically transmit fingerprints to DPS. The list of private businesses is updated regularly and can be found under Fingerprint Information on the DPS website.

Out of State applicants will need to submit hard cards of their fingerprints and results can take 60 - 90 days. If moving to Nevada soon for a new position, please consider waiting to be fingerprinted in Nevada for fastest results.

                    Licensure by Reciprocity

If you are licensed in another state, you may be eligible for licensure by reciprocity. You will still need to go through the application process, including a fingerprint background check. Licenses from other states do not transfer to Nevada.

To obtain reciprocity in the State of Nevada, your license should be active and in good standing in another state and meet the requirements per NRS 640B.330. Only states that require a fingerprint background check for initial licensure are eligible for reciprocity.

Even though you are still required to undergo a fingerprint background check, your license can be issued before receiving the results. However, if criminal history shows up on your background check that would prevent you from qualifying for licensure, you must appear before the Board and your license may be revoked.

To apply for licensure by reciprocity, please include a License Verification from your current state with your application. A printout of an online verification will suffice if all required information is provided. A License Verification is an official document provided by a state licensing board that includes the following important information: your full name, state, license number, issue date, expiration date, licensure based on, current license status, and any disciplinary action and/or pending investigation. A copy of your current AT license from another state is not an acceptable substitution.

                            Initial Application/Licensure Forms

The Initial Application Packet has all the forms you are required to submit for licensure. The Board encourages payment of licensure fees by credit card to ensure a smooth application process. After reviewing the packet, if you have any questions, please email the Board Office at atrainer@nsbat.nv.gov.

                            Live Scan Private Fingerprint Sites

The Department of Public Safety provides a searchable list of private fingerprint sites that have met the requirements to electronically transmit fingerprints. You will be able to search for a business nearest you using a zip code or address. Fingerprints submitted via live scan provide fastest results.

Please submit the request in writing by filing out the NSBAT Public Records Request Form (below). Send the request to the attention of the Board's Public Information Officer.

You can submit your request any of the following ways:

                                    • Email: atrainer@nsbat.nv.gov
                                    • Personal Delivery or Mail to the Board Administrative Office: 6170 Mae Anne Ave., Suite 1, Reno, NV 89523; or

Requests should be as specific as possible and include the requester’s contact information. The Board will respond to the request within five (5) business days.

If the request cannot be fulfilled within five (5) business days, the Board will provide written notice of that fact and let the requester know the earliest date and time it reasonably believes the record will be available. It may work with the requester to focus the request so the Board can respond as expeditiously as possible.

                                          Fees for Public Records

For a list of business addresses for active licensees the Board charges a fee of $25.00, per the NSBAT Fee Schedule (above). The list of business addresses is for one-time use and does not include emails.

Per NRS 239.052, a government entity is permitted to charge a fee for the actual cost incurred in the provision of a public record. This includes, without limitation, the cost of ink, toner, paper, media, and postage.

The Board will inform the requester of any fees, if applicable, prior to responding to the request.
